Buttock Lift — Lift, BBL or Implants?

A surgical buttock lift removes excess, hanging skin and lifts the buttocks and outer thighs — usually as part of a 360° body lift after major weight loss. It is a different operation from a BBL (volume from your own fat) and from buttock implants (silicone projection). Dr. Paulo Michels performs all three in Abu Dhabi, for patients from Dubai and across the UAE.

Overview

What is a surgical buttock lift?

A buttock lift is skin removal surgery: it excises the loose, excess skin that makes the buttocks sag and re-drapes the tissues upward. It does not add volume — it restores position and firmness where the skin itself has lost its elasticity, most often after major weight loss.

In practice the lift is usually performed as part of a 360° body lift (belt lipectomy): the incision passes low around the waistline and, as it is closed and tightened, the buttocks and the outer thighs are lifted — something no fat transfer or implant can do for hanging skin.

That is the key distinction. A BBL adds shape with your own fat; implants add projection with silicone; a lift removes skin. Which one you need depends on whether your real concern is volume, projection or loose skin.

The key decision

Buttock lift, BBL or implants — which one do I need?

The three operations solve three different problems. This is the honest comparison Dr. Paulo Michels walks through at consultation.

Three routes

Who suits which option?

Each route has a clear profile — and each has its own dedicated page.

{{ic-recycle}}

The lift — after weight loss

For skin that hangs after major weight loss, from diet or bariatric surgery. Performed as the back half of a 360° body lift, lifting the buttocks and outer thighs as the waistline incision is closed.

{{ic-heart}}

The BBL — your own fat

For buttocks that are flat or deflated but where the skin quality is good. Fat is sculpted from the waist, flanks and back with VASER and grafted only in the subcutaneous plane under real-time ultrasound — defining the waist at the same time. See the BBL page.

{{ic-shield}}

Implants — for slim frames

For very slim patients without enough donor fat. The most natural route is a composite gluteoplasty: a silicone implant for lasting projection, with your own fat grafted over it to blend the edges. See buttock implants.

The honest science

Why a BBL cannot lift hanging skin

Volume and skin are two different problems — and each has its own fix.

{{ic-heart}}

Fat adds volume — it cannot lift skin

Fat transfer reshapes and fills, but skin that has lost its elasticity does not retract over new volume. Where an apron of loose, hanging skin is the problem, only excision — a lift — corrects it.

{{ic-recycle}}

A lift restores position — not fullness

The lift removes skin and re-drapes the tissues upward, but it adds no volume. If the buttocks are deflated and the skin quality is good, the answer is a BBL — and when both problems exist, the plan is discussed openly at consultation.

The right operation starts with the right diagnosis: skin, volume, or both. That assessment — not a preference for any one technique — drives the recommendation.

Recovery

What is recovery like after a buttock lift?

When the lift is performed as part of a 360° body lift, recovery follows that operation’s staged, supported plan.

  1. Day 0–2

    One to two nights in hospital. You walk from day one; drains and a compression garment are placed, and ultrasound-guided nerve blocks ease the first days.

  2. Week 1–2

    Drains are removed as they slow. Most people return to desk work around two weeks.

  3. Week 3–6

    Garment worn continuously; no heavy lifting or straining while the closure consolidates.

  4. Week 6–8

    Exercise builds back to full. The lifted contour is already clearly visible.

  5. Months 6–12

    Swelling fully resolves and the scar matures and fades into the underwear line.

The other routes recover differently: after a BBL, direct sitting is limited for around two to three weeks to protect the grafted fat; after buttock implants, for around three weeks. Details are on their pages.

FAQ

Buttock lift, answered

Is a buttock lift the same as a BBL?

No. A buttock lift removes excess, hanging skin and lifts the tissues — it adds no volume. A BBL (Brazilian butt lift) adds volume and shape using your own fat, harvested from the waist and back, and does not remove skin. They solve different problems.

Does a buttock lift add volume?

No — it lifts and tightens. If volume is also wanted, the fat-transfer and implant routes are discussed at consultation, so the plan matches both the skin and the shape you are looking for.

Do I need a buttock lift or a full body lift?

After major weight loss the loose skin is rarely limited to the buttocks — it usually circles the body. That is why the buttock lift is performed as part of a circumferential body lift, which removes the belt of loose skin from the abdomen, flanks and lower back and lifts the buttocks and outer thighs in the same operation.

Where is the scar?

The scar runs low around the waistline, like a belt, positioned to sit within underwear and swimwear. It fades progressively over 12–18 months.

Can a BBL fix sagging buttocks?

A BBL can improve a deflated shape when the skin quality is good, but it cannot correct hanging skin — fat cannot lift skin that has lost its elasticity. Truly sagging skin needs excision: a lift.

Who should choose buttock implants instead?

Mainly very slim patients who do not have enough donor fat for a BBL. The most natural version is a composite gluteoplasty — a silicone implant for lasting central projection with your own fat grafted over and around it to soften the edges.

When after weight loss can I have a buttock lift?

Once your weight has been stable for several months, at or near your target, with good nutrition — that is what gives the best and most durable result.

I live in Dubai — where does the surgery take place?

Consultations and surgery take place at Elyzee Hospital in Abu Dhabi, where Dr. Paulo Michels operates exclusively. Many patients travel from Dubai and across the UAE; consultations and follow-up are planned around the drive.
