How much fat survives after a BBL?

The short answer: not all of it — and no honest surgeon can promise an exact percentage. Some of the fat grafted in a BBL is naturally reabsorbed during the first months; the fat that survives is permanent. How much settles for good depends on how gently the fat was harvested, where it was placed, your aftercare and a stable weight.

Why is there no single survival percentage?

Because grafted fat is living tissue, not filler. Every transferred fat cell has to pick up a new blood supply in its new position, and how many succeed varies from patient to patient and from technique to technique. Published figures vary widely depending on how the fat was handled and how the volume was measured — which is why a clinic advertising one precise, guaranteed number should be read as marketing rather than medicine. What a surgeon genuinely controls is stacking the conditions in the fat’s favour.

What happens to the fat in the first months?

In the first weeks the grafts are at their most fragile while they establish a blood supply, and swelling hides the true shape. A portion of the fat is naturally reabsorbed during these early months; by around week six to eight, the fat that has survived is stable and permanent. Swelling then resolves gradually, and the final, settled contour appears between months three and six. So what you see in the first days is not your result: some of that early volume is swelling, and some is fat that will not stay. Good planning accounts for this from the start.

What does the surgeon control?

What do you control after surgery?

Is the result permanent?

Yes — the fat that survives is permanent, living tissue that stays with you. That is also why weight stability matters more after a BBL than after an implant: your result is made of you. Dr. Paulo Michels performs ultrasound-guided, subcutaneous-only BBL surgery at Elyzee Hospital in Abu Dhabi, for patients from both Abu Dhabi and Dubai. Cost reflects factors such as the number of donor areas sculpted with VASER and the extent of the surgery — never promises about survival percentages.

The honest version

Distrust guaranteed numbers

No surgeon can measure your fat survival in advance, and a “guaranteed” percentage is a warning sign, not a reassurance. What can be guaranteed is the method: a gentle VASER harvest, careful purification, subcutaneous-only grafting under real-time ultrasound and realistic volumes — followed by the protected weeks that give the fat its best chance to stay.

Frequently asked questions

Why can you not sit normally after a BBL?

Direct pressure in the first weeks can compromise grafted fat while it is establishing a blood supply. Direct sitting is limited for around two to three weeks, using a special cushion when sitting is necessary, and then normalises gradually.

Does weight loss shrink a BBL result?

It can. The fat that survives is living tissue and behaves like the rest of your body fat, so significant weight loss can reduce the volume and significant gain can increase it. A stable weight keeps the result at its best.

When do I see the final BBL result?

Between three and six months. The surviving fat is stable by around week six to eight, but swelling takes longer to resolve fully, so the final settled contour appears over the following months.

Can more fat be added later if I want more volume?

If more volume is wanted once the result has fully settled, a further session of fat grafting can be discussed at consultation. Planning conservative, realistic volumes first is what protects fat survival and safety.
